Non-lethal violence, since for some reason UT doesn't account for it, does not affect your LV to a certain degree. For example, if you do beat up a couple of people, but otherwise don't actually kill them, then your LV does not increase. But if you just randomly beat people up without killing them over the course of a while, or just go and beat up a large group of people, then your LV WILL increase.

Accidental kills, being mere accidents and not something that you could be judged for, don't count towards your LV. Environmental kills, however, depend on intent. Say you are on a moving train, like you said, and push someone. If you didn't mean to kill them, then let's say you push a little too hard and they fall out a window or something. Your LV will not increase because of this accident. Now, if you INTENDED to push them off a moving train, then your LV will increase accordingly.
