If the new appearance you want also comes with a stat change, then you'll need an alternate form. 

If the new appearance is very minor (e.g. a hair colour change or something like that) then you can probably get away with having it as fodder (just ask first).

If the new appearance is something that fits the rules for shapeshifting, then you can do that and have it permanently. However, note that for shapeshifting:

Additionally, being hit with enough pain may cause your shapeshift to 'glitch' – this won't actually knock you out of the shapeshift, but can reveal you to be a fake, for example by causing colours to flicker. 

So, basically, if you want a new form and appearance then I'd just get an alternate form. Unless the change is only a minor appearance thing.
