Yes. Extra limbs require a move. Want physical wings (i.e. ones you can hit people with)? You'll need a move. Want an extra set of arms, a tail, or a bunch of tentacles? You'll need a move. Anything outside of the standard "two arms, two legs" thing requires a move. As long as you include the rough length of the tentacles as well as any special properties they may have and fancy things they might be able to do (compared to a normal arm), you should be good.
