Power ups with additional effects

When Power Ups are used, is the stat increase the only thing that is allowed to happen? If a power up has an additional effect in-universe (such as causing damage or changes the terrain in a non-fluff way), how is this translated to the move, or it's cost?

For example, there's an item in Doom called the Bezerk Pack, which increases the player character's strength, making it an obvious Power Up. But it also complete heals the PC. If DG has Bezerk Pack as a  Power Up, could it also heal at the same time (I assume the cost for the heal would also apply, I'm not trying to wriggle my way out of that)

Not without having the Heal power or consumables.

Well, I presumed that I would require the healing power.

Ah, okay.

Then yea, it might be something you could do. You might still need a (super) move or something else, even if it's just something that's only activated/triggered whenever you use the power-up.
