As you all know, the Omniverse has a decently large, but not enormous userbase. Which is a good thing, really, because it means that you're not one among millions
And, unlike in MMORPGs where hardcore players jump on a new quest/area/instance the minute it's released and complete it within the first hour of release, there's content that has not been explored yet, things that haven't been done, epic feats and achievements that have yet to be achieved.
So, I want to keep a list of the "epic", notable feats, and who was the first to do so. Note that it's not the same as the Omniverse achievements by Proto Man (which are much more awesome, anyway). If you know someone who has achieved something amazing, of a feat that has yet to be accomplished but which would be epic or you want to correct any entry, do let me know and I'll adjust the list accordingly.
Who was the first to unlock the Void Gate in each of the eight verses?
The Pale Moors: Shang & Atelos
The Tangled Green: Dem secondaries in the saga.
The Vasty Deep: Doomguy seems to be working on that?
The Endless Dunes: Caira!
The Ashen Steppes, Frozen Fields, Dataverse: None yet?
Who was the first to meet Omni?
Probably the same as the first to open a void gate? Shang & Atelos? Or someone else?

Noncanonically there probably were lots, because Diablo n whatnot.
In a graded fight, it was Retane (killed by Gouki).
In all fights, graded and pseudo, it's unclear: Ganondorf (killed by Enel) or Retane (killed by Gouki)?
Who were the first dragonslayers?
Well, the Rathalos is actually a Wyvern, not really a dragon, but okay. The Camelot group that slew the rampaging Rathalos earned that title!
Who was the first to own an Artifact?
Ganondorf!

Ganondorf, again!
Who was the first Staffer, besides Greg/Omni?
You guessed it, Ganondorf!
Who was the first Faction Founder?
If we don't count site NPCs, Ganondorf! He be claiming all the titles.

LAW and New Babylon! Started by LAW... which is Ganondorf again.

Shang and Atelos were indeed the first to meet Omni, following their Moors quest. No one else has done it in the Moors.
Caira found the void gate in the Dunes and is currently chatting with the big guy.
First ever Fusion was Mickey and Proto. First 4-way was with Shang, Atelos, Thaal, Magus.

While it wasn't in an official fight, the first fusion was between Nealaphh and Tartaros in the Vasty Deep as they fought off a shark monster.

One is banned and the other is vanished into the ether, but Gouki and Retane were the first people to have an actual judged fight.
That also made Retane the first prime to be killed.
Ganondorf was the first person to 'win' an artefact.

Ganondorf was the first Prime to be killed, I believe. In a pseudo-fight with Enel. If not, Retane beat me out by just a bit. Same for the fight, really. It was graded, technically, but the end was predecided.
Ganondorf was the first PC with a Base and... technically the first to make a Faction, though the IC stuff for it didn't really kick off until after a lot of other Factions were already in place.
I was the first Staff Member.
The LAW was the first Faction to attack another Faction.
Alex is correct that Ganondorf obtained the first Artefact.
Violet was the first Banishment, via weird Saga shenanigans. I think Stephen Donnel was the first to be Banned in a Graded Fight.

Yea, the Ret-Gouki fight took place about a month, month and a half before Enel-Ganondorf.

I think I used the first Transformation with Ganon in the Rathalos event.
Super Move? I'm unsure. The first one MIGHT have been Gilgamesh with EA in DA.

I used a super move during the Tarrasque event, but I feel like there had to have been something before then.

Just gonna add this: if you can think of something that has not yet been done (as far as you know) but which could go on that list, do say it! Perhaps we can create some incentive for writers to write awesome stuff, and hunt after those open achievements.

Double edit: you have to be past a specific level to to buy a T3 supermove. I'm sure I'll get there but I have yet to accomplish this.
(The earned OM required is 106k.) Earlier I was simply stating my excitement to possibly be the first.
